pnccPrenatal Care Coordination (PNCC)

Did you know that PNCC stands for PreNatal Care Coordination? This is a FREE educational program offered to pregnant women who are covered by BadgerCare+ (state insurance).  The CHD nurse can help connect you to services!

The PNCC program:

  • Helping you find a doctor
  • Preparing you for each stage of pregnancy
  • Preparing for labor and delivery
  • Providing breastfeeding resources
  • Sharing access to a smoking cessation program

Why should I join a PNCC program?

Pregnancy is a time for change for every woman. This program is designed to fit your indivdualized needs and concerns throughout your pregnancy.  A home visitor helps you to plan for a healthy pregnancy and birth, get ready for the new baby, and adjust to the new life as a mother.

When can I join the PNCC Program?

You can start at ANY time during your pregnancy.  If you are a pregnant woman and are eligible for, or already are covered by, BadgerCare+ (state insurance) then you may qualify!
